Requirements: High School Diploma or equivalent. Graduate of an LPN program. One year mental health experience preferred but not required. Valid NJ LPN license.
Responsibilities: Provide case management services to medication only clients which include referrals to external social services and health care resources. Complete discharge summary for medication only clients. Assists psychiatrists and APN’s to maintain appropriate documentation in the medical chart. Serves as a client advocate. Provides intramuscular injections of psychotropic medications to clients as ordered by psychiatrist or APN. Checking client vitals.

Children's Services: Homestead Residential Counselor

Responsibilities: Provide direct supervision to residents.

PACT II: Psych Rehab Counselor

Requirements: Bachelor’s Degree in Social Work, Psychology or related field. A minimum of two years working with individuals diagnosed with a chronic persistent mental illness.
Responsibilities: Participate in a multi-disciplinary treatment team. Assist consumers with activities of daily living through modeling, direct assistance with ADLs and coaching. Provide transportation as necessary. Under supervision, assist consumers in obtaining basic needs. Assess consumers emotional well being, clinical symptomology, behavioral changes, and medication effectiveness.

Project Recovery: Substance Abuse Counselor, CADC intern

Job Id: 056-022

Department: Project Recovery
Requirements: Bachelor’s Degree in Social Work, Psychology or related field. CADC or an approved candidate in the end stage of obtaining.
Responsibilities: Provide case management to assigned caseload. Develop the Individualized Recovery Plan and conduct reviews based on organizational standards. Facilitate skill building, psycho-educational groups to program consumers. Provide crisis intervention as necessary. Participate in a multi-disciplinary treatment team for collaboration of consumer care.
